About this Training Course 

This 2-day course will introduce you to a qualitative methodology to undertake Human Reliability Analysis (HRA) – sometimes known as Safety Critical Task Analysis – which is an approach used to pro-actively assess safety critical tasks and other business critical tasks to identify realistic potential human errors so as to take steps to reduce the probability of their occurrence.

Participants will be taken through a Human Reliability Methodology, starting with capturing and decomposing a task to understand its content and context using hierarchical task analysis, through to identifying potential human errors and relevant actions to reduce human error.

The course will include practical activities to embed and provide experiential learning. Assessment templates are provided for you to take away and use within your own organisation.

At the end of the course, participants will be able to:

  • Describe the reasons why HRAs need to be undertaken
  • Describe the importance of identifying safety critical tasks and why they should be prioritised
  • Explain hierarchical task analysis, including selecting appropriate steps to decompose to a relevant level of resolution
  • Walkthrough concepts of Human Error types, Performance Influencing Factors, explain why each type of human error type can occur and what are appropriate actions to reduce the probability of human error
  • Propose a methodology of undertaking Human Reliability Analysis, including generic Performance Influencing Factors Analysis as a pre-cursor to the actual HRA, and pro-formas for undertaking the analysis

Your expert course leader is a qualified Chartered Engineer with wide-ranging experience in management, manufacturing and maintenance gained in delivering performance improvements in process and manufacturing industries, including for notable blue-chip companies.

He leads an active human reliability consultancy, having completed over 15 assessments for clients that have been accepted by the UK’s Health and Safety Executive as part of a site’s COMAH submission. He has delivered over 25 training courses for over 200 candidates on behalf of the UK’s Chemical Industries Association (CIA), the UK’s Chemical Business Association (CBA) as well as in-house for clients.

In addition to his Human Reliability Analysis training, he has developed and delivered training courses on Continuous Improvement, Process Safety Performance Indicators, Human Integrity, Standard Operating Procedures, and Root Cause Analysis. These are frequently delivered on behalf of the UK’s Chemical Industries Association as well as for the UK’s Chemical Business Association (CBA).

As a result of his work, he has published several papers and articles on continuous improvement, root cause analysis, maintenance, and energy management. He has also been invited to speak at international maintenance conferences, has lectured at the National Centre of Tribology, and presented papers at the Hazards and HazardEx conferences.
